92 ford f150 with 5.8 starts very hard first time of day (coughs spits and sputters )after some fighting it it runs after that it runs perfect starts perfect

I would check temperature sensors. Coolant, intake air. Check Evap valve and hoses between canister and intake manifold. Check EGR valve. Isolate them one by one, make them work,
You must clean the fuel injection with fuel injection cleaner and check your vacuum hoses if all is good is your check engine light on? to verify that the bulbs not burned out turn the key to the on position all the dash lights should be on // oil brake check engine if you don't see it on the bulbs burned out there should be codes set go to autozone and have them read if there's none then either your throttle position sensor is bad or the idle air control valve is shot
