Do Catatlytic Converters alwasy need to be replaced? Can they be cleaned? I have a 2009 Lexus ES350 with 57k miles. It seems odd that they would fail with that mileage

The filtering inside of the converter basically melts causing a back up of exhaust gasses or inefficiency with the car's intended purpose.
