The MOH-USB adaptor is a linux-specific USB sound card device, and only works for Cisco's CallManager product.
If you have a CallManager installation, and you're trying to add Music-On-Hold, you can use this adaptor to bring in a fixed or live audio source via 3.5mm or RCA jack.

If you don't have a CallManager installation, this adaptor will probably not do anything for you. I'm not aware of any windows drivers for it, nor any non-proprietary Linux drivers.
I don't have a picture of one, and google was no help in the search for an image. I was only able to get confirmation that the sound input is a line-level signal, so it is most likely a 3.5mm jack like a standard sound card.
I did however find a message board thread here that seems to indicate that you can get MOH with non-cisco USB sound cards in some versions of Call Manager.
