Also, truck has had a few instances over the last few months where when I put it in reverse it wouldn't move right away when I applied the gas and when I applied more gas it would drop into reverse with a loud, hard clunk and lurch backwards abruptly.

Take to a transmission shop for a diagnosis , should have gone a bit sooner before getting thes eother codes etc
Sorry Melanie , you were right to post on fixya as sometimes there are quick fix's that are relatively inexpensive but auto tran's are not usually one of them Sorry if i disappointed you
