The entire system is under pressure. In some places there is higher pressure than in others. Even your water surface is under the weight of atmospheric pressure. But more serious, when water gets hot it expands and tries to escape down it's only escape route. In this case a pump helps it further along thus increasing the pressure. There does not have to be a blockage anywhere for an old hardened plastic tube to not fit snug enough anymore to assert enough friction to not pop off a connection joint. I experienced this on the rear washer nozzle of my wife's hatchback. So check your pipes. If they feel hard or look old, replace them.
I assume that you have taken the sliding front bit with the outlet apart and checked that there is no blockage there and the machine was regularly cleaned as per service instructions.
Sounds like a blockage somewhere, and the build up of pressure is blowing the pipe.
