There is a box that will fit through a 4 inch hole that you should consider ... it works like a screen door brace (but much heavier) you put the 'bolt' through the hole and tighten it up against the ceiling joists. Then, you put the screw on bracket over and under the bolt ... then you can fasten the box to the bracket. I could do chin-ups on this box ... if I could do chin-ups. I did hang from it. It will definitely suppport a fan if it will hold my 205 frame. Expect to pay about $14.00 and find it at good hardware stores or at the big box place.
