Have done more research on this, and made some tests. I found the Ext Speaker jack sometimes did not make good contact when the earphone plug was removed, and cleaned/lubed the jack ... the entire unit must be disassembled for this, as the jack is mounted on the third circuit board which is soldered to the antenna jack and a ground shield in several places.
Audio output to an external speaker, whether connected to the earphone jack or directly to the main speaker contacts, is the same. An unpowered external speaker (4-inch audio speaker was used) has quite satisfactory volume. The internal speaker audio works, but seems distorted - I'll try to find a replacement.
I replaced the speaker with a new one from iCom - only $11 with a few dollars for shipping. Distortion is gone now - hard to tell whether the volume improved or not.

Check the mode button and make sure you are in the right mode, sounds like you have it in the wrong mode.
